Bus Pass Forms:

  • As of 12 May, 2016, bus pass applications must now be completed online. To access this link, please click here. When selecting the forms please take note to select ‘Travelling outside the Opal Network System’ as Opal is not available in our area. Once the form has been completed online; print, sign and date the form and send into Admin as we will need to verify the form and then forward onto Transport for NSW.   • Private Vehicle Conveyance (PVC) Subsidy Application Forms can be made online. Eligibility criteria applies for children 4 years & 6 months of age or older and residing more than 1.6km walking distance from the nearest transport pickup point.
